Recent data show that more than <strong data-start=\"1195\" data-end=\"1236\">40% of patients discontinue treatment<\/strong> after the first three years, with <strong data-start=\"1271\" data-end=\"1312\">serious consequences for their vision<\/strong>.<\/p><p data-start=\"1315\" data-end=\"1667\">The search for new therapeutic strategies capable of <strong data-start=\"1368\" data-end=\"1410\">permanently or long-term counteracting<\/strong> the pathological effects of exudative maculopathy has led to the exploration of <strong data-start=\"1491\" data-end=\"1518\">gene therapy techniques<\/strong>. These involve delivering <strong data-start=\"1545\" data-end=\"1597\">therapeutic DNA into the patient\u2019s retinal cells<\/strong>, enabling them to produce the anti-VEGF drug directly within the eye.<\/p><p data-start=\"1669\" data-end=\"1935\">This therapeutic DNA would be administered <strong data-start=\"1712\" data-end=\"1750\">in a single intravitreal injection<\/strong>, allowing for continuous production of anti-VEGF in the eye\u2014potentially <strong data-start=\"1823\" data-end=\"1884\">eliminating the need for repeated intravitreal injections<\/strong>, or at least drastically reducing their frequency.<\/p><p data-start=\"1937\" data-end=\"2266\">The <strong data-start=\"1941\" data-end=\"1972\">most promising gene therapy<\/strong> for the potential <strong data-start=\"1991\" data-end=\"2040\">definitive treatment of exudative maculopathy<\/strong> is called <strong data-start=\"2051\" data-end=\"2089\">Ixo-vec (Ixoberogene Soroparvovec)<\/strong>, developed by <strong data-start=\"2104\" data-end=\"2131\">Adverum Biotechnologies<\/strong>. Once injected, Ixo-vec is expected to provide a <strong data-start=\"2181\" data-end=\"2230\">long-lasting and effective therapeutic effect<\/strong> without significant adverse events.<\/p><p data-start=\"2268\" data-end=\"2418\">A <strong data-start=\"2270\" data-end=\"2296\">Phase I clinical trial<\/strong> (named <strong data-start=\"2304\" data-end=\"2313\">OPTIC<\/strong>) and a <strong data-start=\"2321\" data-end=\"2339\">Phase II trial<\/strong> (named <strong data-start=\"2347\" data-end=\"2355\">LUNA<\/strong>, currently ongoing) have been conducted to assess the therapy.<\/p><p data-start=\"2420\" data-end=\"2921\">In the OPTIC study, <strong data-start=\"2440\" data-end=\"2518\">most patients remained free from anti-VEGF injections for over three years<\/strong>, an extremely encouraging result. The subsequent LUNA study is designed to evaluate whether <strong data-start=\"2611\" data-end=\"2631\">lower drug doses<\/strong> combined with <strong data-start=\"2646\" data-end=\"2680\">enhanced prophylactic regimens<\/strong> can achieve even better outcomes. LUNA aims to determine the optimal dose of Ixo-vec and the most effective <strong data-start=\"2789\" data-end=\"2833\">local corticosteroid prophylaxis regimen<\/strong>, with or without oral prednisone, to define parameters for future <strong data-start=\"2900\" data-end=\"2920\">Phase III trials<\/strong>.<\/p><p data-start=\"2923\" data-end=\"3113\">At the <strong data-start=\"2930\" data-end=\"2995\">American Society of Retinal Specialists (ASRS) Annual Meeting<\/strong> held last July in Stockholm, preliminary data from the LUNA study\u2014collected 26 weeks after initiation\u2014were presented.<\/p><p data-start=\"3115\" data-end=\"3665\">The data showed that <strong data-start=\"3136\" data-end=\"3191\">Ixo-vec has a favorable safety and efficacy profile<\/strong>. The drug dose used, lower than in the OPTIC study, resulted in <strong data-start=\"3256\" data-end=\"3277\">less inflammation<\/strong> while maintaining therapeutic efficacy: <strong data-start=\"3318\" data-end=\"3338\">100% of patients<\/strong> experienced either <strong data-start=\"3358\" data-end=\"3398\">no inflammation or mild inflammation<\/strong>, which was resolved with the study\u2019s prophylactic protocol. Moreover, <strong data-start=\"3469\" data-end=\"3541\">76% of patients remained free from intravitreal anti-VEGF injections<\/strong>, maintaining <strong data-start=\"3555\" data-end=\"3579\">stable visual acuity<\/strong>. The therapy also proved <strong data-start=\"3605\" data-end=\"3664\">beneficial for patients previously treated with Vabysmo<\/strong>.<\/p><p data-start=\"3667\" data-end=\"3825\"><strong data-start=\"3667\" data-end=\"3737\">Ixo-vec currently represents the most promising therapeutic option<\/strong> for radically changing the treatment landscape for patients with exudative maculopathy.<\/p><p data-start=\"3827\" data-end=\"4152\"><strong data-start=\"3827\" data-end=\"3854\">Adverum Biotechnologies<\/strong> is expected to release <strong data-start=\"3878\" data-end=\"3932\">9- and 12-month follow-up data from the LUNA study<\/strong> soon.
